Topic: “Food Security and Impact on Seniors: Strategies for Screening and Connecting Seniors to Local Food Resources”.

Presented by: Heather Tant

Heather’s bio: “As a dietitian, I have known the impact of malnutrition on decreased function, quality of life, increased morbidity and mortality and increase health care costs. Seniors are particularly vulnerable to malnutrition due to food insecurity and social isolation. In my current role as Manager for Chronic Disease & Nutrition Therapy been I have been involved in several strategies, hoping to bring attention to the importance of nutrition and affect positive changes in our work including: local site (NRGH) investigator for the Nutrition Care in Canadian Hospitals research; integrating malnutrition screening into the electronic admission documentation (IHealth), and advocating for the inclusion of food security screening in primary care and across care transitions.”
