“COPD: Hospital to Home” presented on February 23, 2021
This education was presented by Carmen, Christine and Laura of the Vancouver Coastal Health COPD Transition Team. Thank you so much for sharing your knowledge and expertise on COPD Care!
The COPD Transition Team started in 2012 and currently they are a team of 2 Respiratory Therapists (RTs) and an Registered Nurse (RN) based at VGH. They see patients who are admitted to VGH with a COPD exacerbation or who are treated for a COPD exacerbation while they’re in hospital for other issues or who are just struggling with managing their COPD and treatments. They follow patients in the Vancouver area from hospital to home for 1-3 months post discharge to try to prevent further flare ups and readmission.
